Seven offices recognized as Patient-Centered Medical Homes


JUNE 2011 -- Group Health’s Anderson, Kenwood, Finneytown, Clifton, Mason, Springdale and Western Hills offices recently attained Level 3 recognition as a Patient Centered Medical Home (PCMH.) Medical Home is a program of the National Committee for Quality Assurance, a not-for-profit organization dedicated to improving health care quality.

The Medical Home is an approach to providing comprehensive primary care for children, youth and adults that facilitates partnerships between patients, their personal physicians, and, when appropriate, the patient’s family. The Group Health Springdale office was chosen to be one of a select group of practices to pilot the program in 2010.

In a set of six standards that describe clear and specific criteria, the program helps practices organize care around patients, work in teams and coordinate and track care over time.

To qualify as a PCMH, practices must meet all six standards, which can result in one of three levels of recognition. Level 3 is the highest. Practices seeking PCMH complete a web-based data collection tool and provide documentation that validates responses.
